Primary Care
Dimensional Health can be your first stop for current and ongoing care. We provide comprehensive primary health services including annual physicals, woman wellness visits, ongoing care for chronic conditions, treatment for sudden illness, and specialist services. Appointments range from 30-60 minutes, allowing us the time to take a holistic approach to your health.
Physical Therapy
Functional movement is a key to enjoying your life. Let us help keep you active by working with your very own body mechanics.
Secondary Care Navigation
We guide our members as they experience care beyond the four walls of the clinic. Consider us your trusted advisor as you navigate a complex health system.
24/7 Access
Our commitment to your health goes beyond business hours. Our physicians are available by phone to our members after hours, weekends, and holidays.
Health Coaching
Get help with sleep issues, meet fitness goals, or learn to eat healthier. We can also help you quit smoking, find better work-life balance, and stay at a healthy weight.
Mental Health
Our behavioral health therapists use integrated, evidence-based treatments that are easily accessible so that we can work together on connecting to your values, learning coping skills, making healthy mental changes, and helping you thrive.
Same-Day Services
We develop our daily schedule to ensure that we can handle a few extra appointments per day. This allows our nurses or doctors to take care of urgent needs of our members.
Labs
At Dimensional Health we offer lab tests inclusive of your $55 office fee or $25 office copay for our members when ordered by your Dimensional Health PCP. If your provider orders additional labs which must be sent to an outside lab, you can expect these labs to be covered as part of your office visit fee.
